In future if a new Layout Based Visit Form is needed, see this.
Attachments 3 & 4 refer to the LBV Form, which is very useful for specialties which don’t have forms for their writing clinical notes.

Another reason is to refine Wiki articles if there are areas of confusion.
It has become very rare that questions/problems are not addressed within 2 days.
We don’t have a 100% resolution rate, but we do try to respond in a timely fashion.
