Amendment can be used to make it explicit as to the identity of the signer.

Or it can be worded that document was written by medical student & countersigned by attending.

Another option is to have additional e-signs. Sometimes a note is signed by the medical student, the first year resident, the second year resident, the chief resident & the attending.

Downside is that every singature is sued if malpractice is alleged.

A report of forms: Not Signed, Signed but not locked, Signed and Locked
