Medicare is requiring that all of our reports have Name, DOB, and diagnosis printed on each page. Currently the output has Name and DOB on the header. Does anyone know how to add diagnosis to header?

Specifically which report? are you talking about the Patient Demographics -> Report link?
If so how would you know which diagnosis to put in the header, there could be dozens, every encounter and whatever is in the Problems/Issues list.

For MU attestation the Clinical Summary & the Transition of Care document must contain diagnoses, but there is no requirement for the location of the diagnoses.
2015 attestation is moot at this point.
