HCFA 1500 box 13

I am moving to a fee for service model. I would like to give my clients CMS 1500’s to send into their insurance after visits. I would like to have Box 13 blank so that they are reimbursed directly and not me. How would I do that?

think you could still complete box 13 with signature on file if you choose to not accept assignment in box 27
