Total Cholesterol
LDL Cholesterol
Smoking status yes or no
If yes: Smoking cessation offered yes or no.
Colorectal cancer screening, What type was it? Hemoccult, rectal exam, flexible sigmidoscopy, colonoscopy.
Females over 50 – mammogram
High risk of cardiac event:
On aspirin yes or no
Received flu vaccine yes or no
Medicine reconciliation

I have started working on a simple Laboratory Information System and are going to leverage off of that.

Jeremy Wallace is looking for a GPL’d report engine to help graph the output of reported percentages, blood pressures and lab data.

Maybe in the administration Lists page, we could designate certain forms as being mandatory and an encounter would be flagged as incomplete until these forms were completed.  Then we pre-install mandatory forms with the above info, A1C, BP, LDL etc…
