Jason, I know you have been trying to get a WYSIWYG type of Form Creation tool… and that’s great!  I have been playing with the current Form Maker script developed by Dr. Leeds and am having issues with Checkboxes and Popup Menu’s.  It seems that the data is being stored into the database but when you use go to view the form after creation the Checkboxes and popup menu information is incorrect.

I am using Form Creator v3… any ideas and/or has someone fixed this issue?

By the way, I have a more detailed howto for custom forms that I made to help Windows users create forms. The only platform specific information is the Strawberry PERL instructions, so anyone can use it. I know that it’s not a substitute for a WYSIWYG form creation tool, but it helps people who need a more detailed guide than the one provided at oemr.org. Here’s the link.

Chris, you may want to check out the new support for "layout based" encounter forms in current CVS.  Not WYSIWYG but very useful and flexible.

Nevermind Rod, I see what you are referring to.  You are talking about the layout for demo, reports, etc.  I believe that is in v.3.1.0 as well…

Is the mapping from the encounter forms to use the layouts in there already?

The support for layout based *encounter* forms is only in CVS, not in 3.1.0.  See the thread "Layout Based Encounter Forms" also in this Developers forum.

Sorry I didn’t respond sooner.  I have not been checking the forums as often lately.  I thought the problem with checkboxes being off was fixed.  Can you post the contents of your form’s text template or an example that produces the same problem and a description of exactly what goes wrong?  I would be happy to fix the problem.
