Is it possible to set default CPT4 and ICD9 codes that will appear on a new encounter fee sheet for patients who have multiple encounters (over a few weeks) for the same condition(s)?

  For this you may want to use CAMOS.  Within CAMOS you can pre-set billing codes as well as text that is easily selectable.
