I would leave the ‘codes’ table alone, and add a new column to the ‘insurance_companies’ table - some kind of %-age factor to come up with different fee schedules for different payers. Of course, the code has to be changed in the appropriate places.

In a management related issue, we are usually advised to have only one fee schedule and then write off the "disallowed" amounts.
The thinking is that if you through chance bill 51% of your charges at a lower fee for a particular CPT code, the insurnce companies will lock you into this as the "reasonable and customary charge" for that CPT code.
Recovering from this can be a real problem. You are only allowed to increase your reasonable and customary fee by 1% per year. A fifteen percent discount would take almost 15 years to recover.
Simply, better to have only one fee schedule.
