Please check the “Previous Names:” section under “WHO”. It seems by default all names are selected.
Please select any appropriate value or any one value - so it will work.
We guess there should be more values in that box - which is preselected - which causes the issue.
This field has bug - which is being discussed & fixed in this thread - which will be out in next patch or you can do a temporary fix discussed in the thread.
