Encounter Report Dates are missing

Before the latest update to 4.2.2 everytime I go to Patient report I was able to select each encounter individually but now everything is all together.

The first picture was the current situation. The second picture was before where each encounter has a date next to it and was separated individually. I’m not sure if the update has anything to do with it. It was just happened during the same time period.

All the previous encounters are still separated. But since now, everytime I created an encounter it does not show a date and all encounters are lumped together.

Please help me get them back to it was before.

Hi Nhat,
We just checked with regular OpenEMR version 4.2.2 and was not able to get the issue.

Give us more details on the following:

  1. Was the EMR updated from a lower version to a higher version. If yes, share the older version number.
    (or)
  2. Is the EMR updated after a local fix or a code change based on customizations.

Give us these details. We will try to get this solved.

1, Yes it was updated from version 4.2.1
2, No, I did not made any changes on coding

I think the problem could be from checking in the patient. When I use the “Calendar or Flow Board” to change the status to Arrived and Checked out it would gives an Encounter date and Reason for visit.

When I manually create an encounter from “Encounter history”. It does not create an encounter date and reason for visit. And all the encounters would be lumped together.

I’m not sure if that is the same with your system. But I thought no matter which method to create an encounter it should be separated. Or else it would be very difficult to select each one.

Edit: I tried a to use a fresh installation of the program and even when I manually created an encounter it separates them by dates.
