Custom Form DB error

Hi,
I am trying to create a custom form by following the steps given in exapmple2. I have created the new.php, view.php, save.php and table.sql files in the formmaker folder itself.

The form that I have created is also in the formmaker folder with the name “my_form.txt” . Now in the command prompt when I give formscript.pl my_form.txt it was executed without any errors and I was able to seemy form in the Un-Registered section under Forms in OpenEMR. Now when I try to say Install DB (after registering the form) I get the error message
“ERROR: could not open table.sql, broken form?”.

Can any one please tell me what am I doing wrong?

hey,

What OpenEMR version are you using? Would be best to see the files. If you have a github/git account then would upload there, and ask us to look at them. Otherwise email a zip package of them to brady@sparmy.com and I’ll throw them on my github repo (note this may take a bit, since my free time is limited).
