Computerized Order Entry Repots

Hello everyone. Sorry to be pestering everyone, but am trying to get things right. For things like CPOE, or for transition of care that meet requirement (i.e. including vitals, procedures, demographics, care plans, etc.) where would these reports be located? Again, could someone give a screenshot of what it should look like? I’m trying to import data into the system and not only make sure it goes in, but that I can generate a correct report. Any help would be appreciated.

To clarify what I am asking about, I am looking at this document: https://www.cms.gov/Regulations-and-Guidance/Legislation/EHRIncentivePrograms/Downloads/TableofContents_EP_Medicaid_ModifiedStage2.pdf and how to make sure everything is set up to complete this.
