CMS 1500 PDF Blank

Good day,

We are running oemr 5.0.2, and using custom codes list instead of built-in cpt & icd9. Now, when I generate the CMS 1500 PDF returns a blank page.

What might be the cause?

Found out that CPT4 code has to be present for the CMS 1500 PDF to be generated :wink:

We have the CPT4 codes in and have been active for a long time and the form is still coming up blank.

Once we activated the CPT4 and added them to the fee sheet we could generate the hcfa pdf.
