CMS 1500 PDF and Text problem

For some reason today I am just getting a blank screen when I hit the CMS 1500 PDF and Text buttons. Where would I look for the file that generates these?

globals > Features > CMS 1500 Paper Form Format shows 2/12 selected. I looked in C:\xampp\htdocs\openemr\library and gen_hfca_1500_02_12.inc is there but the button does not generate the form. The X12 button works fine. Any ideas?

I loaded the new 6/19 patch and the problem was solved.
