We are a behavioral health clinic and we are trying to edit the issues form to better serve us. I can’t figure out where the layout is for this section. Its a tab on the front page by the document tab and transaction tabs. Please help.
Thank you. Just some advice. Our entire treatment revolves around the Treatment Plan. I was thinking of adding the treatment plan questions to the history section of demographics and then making a layout with those fields or trying to do the same with the issues section. Not sure if this is even possible.
e.g. Problem1:
short term goal
long term goal
objective1
objective 2
Then when I am doing a patient note I can have these answers fill out on the patient note automatically. Does that make sense?

It is possible to add additional fields to history tab in demographics screen by using following steps
1.Navigate to Administrator->Layouts.
2.Select history from Edit layout drop down.
3.Add the fields that need to be added under any sub tabs(General,family history,others) that you wish to add.
4.Save the changes and you will get the fields in history tab under respective sub tab
To auto populate the data from history tab to patient notes there is need of code customisation.
