Can't add claims for new clients: insurance company doesn't fill in

My client has used openEmr successfully for many years - billing using x12

Recently she has this problem: she gets a new client and fills out his or her info as usual, including adding in the insurance company. When it comes time to fill out a claim, however, the insurance company field does not fill in, and in fact when she clicks on the dropdown the list of insurance companies does not appear . This results in a blank in the x12 form and so the claim can’t go through.

We tried deleting a handful of old clients and creating new claims, but that didn’t help.

And she is able to successfully bill for older clients.

Is one of the databases too full? Corrupted?

I’m using OpenEMR version

Where do I find that information? To be honest it hasn’t been updated in a number of years, I guess. Am a little nervous to change it, in case it causes more problems.

Did you check the logs?

No - I will check later, but wondered if this was a known problem.

Sorry, but how can I tell which version I am on? Wasn’t able to see.

Unless it auto-updates, I don’t believe we’ve upgraded for a few years by now. If that’s the case, do you think I’m in any danger of losing data if I do upgrade?
