Cancel does not work in 4.0 user and Facility

Hi
  I just updated my cvs install via cvs-openemr.deb.
In Edit Facility the Canel button does not work.
And in Add or Edit user the Cancel it no work either.
In Practice they all Cancels work just find.

i can confirm this bug report on the 'add new issue  form (patient_file/summary/add_edit_issue.php) on the current openemr 4.0.0-dev demo.

(note they include aperezcrespo’s bugs; Justin, I was unable to duplicate yours in the tree view layout(goinf to it by clicking ‘manage’ from the patient summary screen), what layotu you using?)

i just checked it out on the 4.0.0-dev demo site. at the time, the site appeared to have the old radiobutton layout.
