I accidentally used a function name that is in one of the included files.  For some reason, this error did not appear on the system I was working on at the time.  You can download it and overwrite the files if you want or you can go into new.php and change the two occurrences of getPatientData() to getMyPatientData(). 

I am actively working on ironing out bugs and adding to the documentation, so there is now a line on the website below the link to tell you the date and time of the last update of the file.

I am also working on a set of starter data for the columns that is more complete.  This has been a complaint since the beginning.  This should be done soon and make it easier for new users to get started.

That did it.  Changing getPatientData() to getMyPatientData() solved the problem.  Now to follow the previous instructions.

The update has better sample column data.  you can get it from my site or cvs.
