Box 11 of the HCFA

We are missing box 11 data for the HCFA. The insurnce polocy number is not showing in the x12 file generated. We were told this by Availity.

Full Message:

“Courtney emailed me this morning from Availty and said that the 5010
format looks good except it is not pulling the patient’s insurance ID
number to the claim form. Would you ask Sherwin to make sure that that
is mapped to box 11 of the HCFA form”

Has anyone addressed this issue yet?
I did check the misc billing form and box 11 is not there.

I noticed int he code that the insurance inforamtion was connected the provider information. I am not going to pretend to understand why but what are we missing got get the inforamtion to show on the form.

Box 11 is the Group Number not the Policy Number. Make sure the user has a Group number entered.

They do and I had to go back and examing the output and it is the group policy number.
I just figured that Courtney is looking in the wrong box for the policy number. it is in box 1a not box 11.

I will upldate them.
