Blank CAMOS entries on XAMPP windows

Hi,
  I just installed OEMR4 with SP2 on VISTA with XAMPP.  All works well except CAMOS.  I can create and modify categories and add text just fine.  But when submitting to the encounter all seems well but the  CAMOS text does not make it into the encounter.  The CAMOS Entry title is in the encounter but not the text.

Any Ideas?

I am trying to install open emr 4 and I have used XAMPP. Install of XAMPP was ok. Then I copied Openemr folder into the htdocs folder. and used the command htp://localhost/openemr/setup.php then on the first scrren it can see error message like this “Notice: Undefined index: state in E:\xampp\htdocs\openemr\setup.php on line 13” all the remaining part of the screen is also there with the option to continue, please let me now how can fix this error to proceed. I have check mysql and appach is running

What version of XAMPP are you using (both ajperezcrespo and umerrazi)?
In the installation instructions note some specific issues noted in this section: http://www.openmedsoftware.org/wiki/OpenEMR_4.0_Windows_Installation#Required_Software_Installation

  XAMPP  1.7.4 VC6
PHP.INI changes as per OEMR required.
All else works just fine

I was using 1.7.4 which was giving problem, then used XAMPP 1.7.3 but at the end it asks lot of configuration with Y/N options which is confusing part for me. I do not see any instructions for proper selection. Would you please give me the instructions for XAMPP 1.7.3 installation that would help me to select proper Y or N response for those selections.

If nobody chimes in, then may make sense to post the questions here. Then we can figure out the answers (and also place them in the installation wiki page).

Also, ajperezcrespo, note OpenEMR is not considered to be compatible with 1.7.4 yet. Would be nice to get it compatible, though. What errors are you seeing in the php error log when try to submit and look at the CAMOS text?
