There is a known nasty bug when using recent XAMPP/WAMP versions secondary to the audit log engine. If need audit engine, then suggest either:
1. Trying out the development version of OpenEMR 4.1.1-dev, since work has been done to fix this bug (let us know if it works).
2. Using an earlier version of WAMP. Looks like need to use WAMP version <= 2.0 to be compatible according to below install page:
