Given that OEMR 4.2 is not certified for stage 2, if a clinic has attested to stage 1 in 2013 and 2014 and now for 2015 they would be doing stage 2 can they attest to stage 1 again(or modified stage 2) for a third year or can they apply for a hardship extension for 2015?

Flexibility Rule of August 2014 permitted the use of 2011 CEHRT for attestation only for year 2014.
For attestation in year 2015, 2014 CEHRT must be used, irrespective of stage.
Because OpenEMR has yet to receive that certification, there is no choice but to get the Hardship Exception.

OK, looks like the hardship deadline is July 2016 for the 2015 year but they have not posted how to apply. The only info is for CAH’s. Anyone apply in years past? Are they handed out automaically in the case of EMR vendor’s not being ready?

Only automatic exception is for the first year out of post-graduate training.
Everyone else must send in the application form.
