Add a physician or practitioner

dear colleauges,
I am trying to add a physician into openEMR 5.0.1 patch 4. I have 3 facilities already, but when I add a user, it only shows the primary in the dropdown list, instead of show the all in the list and I can choose.

@ebudiahr,
Can you please check whether the service location checkbox is enabled for the facilities you have created. If not please enable it and check again.
