5010:Skip Onset Date if == Service Date

Tested and working on my system.

This addresses the issue Tony Brought up where if onset date and service date are the same, the onset date shouldn’t be included.

You are correct, I should have started an new thread.   I’ll pull these changes in.  This is a rel-410 customer, should not be an issue to patch it.
