4.1/4.2 Vital form blank over WAN

We have OpenEMR hosted in our datacenter and accessible from the internet. We have run into a situation where the vitals form is blank when connecting to OpenEMR from the internet. Local access is fine. Tried both versions as well as different browsers. Any ideas?
