I am experimenting with OpenEMR for use at our practice. One aspect of patient care we wish to pursue is having patients fill out questionnaires electronically. Does OpenEMR provide functionality for this?

Would use the patient portal for that.
The current stable OpenEMR version (5.0.0) has an “offsite portal” which should be able to support this:

And the current development version (5.0.1-dev) also has a version 2 “onsite portal” that should be able to support this:
